– Documentation of complete firefighter mile (within the past year)
– Copy of valid driver’s license
– Copy of high school diploma or GED
– Copy of college diploma (if applicable)
– Copy of CPR card (if applicable)
– Copy of State of Ohio EMT/Paramedic card (if applicable)
– Copy of State of Ohio fire card (if applicable)
– Copy of State of Ohio fire safety inspector card (if applicable)
– Copy of any instructor cards (if applicable)
– Copy of doctor’s vaccination records (Tetanus, Pertussis, Diphtheria, MMR, Polio, Varicella, Hep B)
– Documentation of any specialized training (Hazmat [any level], ropes, confined space, swift water…) (if applicable)
